  • Planning social activities, educational programs, Palliative Care Awareness Week events, and other initiatives aimed at improving the quality of life of patients and their families.

PATIENT AND FAMILY EDUCATION

  • Providing information about the disease process, care methods, and symptom management.
  • Supporting patients and their families in understanding patient rights and participating in healthcare decision-making.

COORDINATION AND COLLABORATION

  • Promoting multidisciplinary teamwork among healthcare professionals, social workers, psychologists, and other relevant specialists.
  • Integrating home care, hospital services, hospice care, and community resources to ensure continuity of care.

END-OF-LIFE CARE

  • Supporting acceptance of the end-of-life process while preserving the patient's dignity and comfort.
  • Providing bereavement support and guidance for family members.
